  1.         UNIFORM LIVING WILL  
  2.           OF Irving Titlebaum     
  3.  
  4.     To my family, my physician, my lawyer, my clergyman. To any
  5. medical facility in whose care I happen to be.  To any individual
  6. who may become responsible for my health, welfare or affairs. 
  7.  
  8.     Death is as much a reality as birth, growth, maturity and old age
  9. -- it is the one certainty of life.  If the time comes when I,
  10. Irving Titlebaum, can no longer take part in decisions of my own
  11. future, let this statement stand as an expression of my wishes
  12. while I am still of sound mind. 
  13.  
  14.     If the situation should arise in which I am in terminal state and
  15. there is no reasonable expectation of my recovery, I direct that I
  16. be allowed to die a natural death and that my life not be
  17. prolonged by extraordinary measures. I do, however, ask that
  18. medication be mercifully administered to me to alleviate suffering
  19. even though this may shorten my remaining life.

  21.     This statement is made after careful consideration and is in
  22. accordance with my strong convictions and beliefs. I want the
  23. wishes and directions here expressed carried out to the extent
  24. permitted by law. Insofar as they are not legally enforceable, I
  25. hope that those to whom this will is addressed will regard
  26. themselves as morally bound by these provisions. 
  27. If it is permissible under the laws of the jurisdiction in which I
  28. may be hospitalized I direct that the physicians supervising my
  29. care upon a terminal diagnosis to discontinue hydration  (water)
  30. should the continuation of hydration be judged to result in unduly
  31. prolonging a natural death.  
  32. If it is permissible under the laws of the jurisdiction in which I
  33. may be hospitalized I direct that the physicians supervising my
  34. care upon a terminal diagnosis to discontinue feeding should  the
  35. continuation of hydration be judged to result in unduly 
  36. prolonging a natural death.  
  37. I herewith release any and all hospitals, physicians, and  others
  38. both for myself and for my estate from any and all  liability for
  39. complying with this declaration, to the fullest extent provided by
  40. law.  
  41. I herewith authorize my spouse, if any, or any relative who is
  42. related to me within the third degree to effectuate my transfer
  43. from any hospital or other health care facility in which I may be
  44. receiving care should that facility decline or refuse to 
  45. effectuate the instructions given herein.
